OK. Usually a damaged header in an rpmdb.

Perhaps cache inconsistency though, try
     rm -v /var/lib/rpm/__db*
and see whether that cures.

Otherwise mail me privately the output of
     rpm -Uvv var/lib/smart/packages/*.rpm

You might also append the output of rpm -qa -vv.
